COMPENSATION AND BENEFITS Physical Therapist (PT)

• $25,000 Start/Retention Bonus
  
• Performance Bonus $10,000 + Relocation Allowance
  
• Retirement Match, Health and Dental Insurance, CME Allowance, 22 Days of PTO/Holiday Pay
  
• Experience Based Salary up to $90,000
  

RESPONSIBILITIES AND FACILITY DETAILS Physical Therapist (PT)

• Clinic Based
  
• Flexible Scheduling Options - 4 x 10 or 5 x 8
  
• Majority Patients are Adult Orthopedic with some Neurological, Post and Pre-Op
  
• M-F scheduling, No nights, No weekends
  

QUALIFICATIONS AND SKILLS Physical Therapist (PT)

• Possession of or ability to obtain Arizona licensure
  
• Bi-lingual English/Spanish a Plus (not required)
  
• Open to any experience level - New Grad to Seasoned
  

COMMUNITY Physical Therapist (PT)
